The sequel opens immediately where the first film ended. Grace is taken to the hospital, where she meets her estranged sister, Faith. However, the hospital is quickly attacked by mercenaries sent by Bill Wilkinson and rival cult members.
Grace learns that because the Le Domas family failed to sacrifice her, the Satanic High Council—represented by Elijah Wood's character "The Lawyer"—has declared a succession challenge. Four global dynasties (the Danforths, El Caidos, Wilkinsons/Chen, and Rajans) are given until dawn to kill Grace or claim the High Council seat.
Throughout the night, the rival families engage in brutal infighting while hunting the sisters. In the final act, Ursula Danforth (Sarah Michelle Gellar) and Titus Danforth (Shawn Hatosy) corner Grace and attempt a legal marriage loophole to absorb her claim to the throne. However, Grace and Faith fight back, turning the ritual into chaos.
As the sun rises without Grace being sacrificed, the remaining members of the rival families explode into showers of blood one by one due to breaking their pact with Mr. Le Bail. Mr. Le Bail briefly appears in person, gives Grace and Faith a respectful nod for surviving the game, and vanishes, leaving the sisters alive as the sole victors.
