- Season 1 Twist: Sherlock’s presumed dead lover, Irene Adler, returns alive and is revealed to be the criminal mastermind Jamie Moriarty. She orchestrated Sherlock’s London breakdown because his intellect threatened her criminal syndicate. Joan deduces Moriarty's psychological fixation on Sherlock, orchestrating her capture.
- Seasons 2–3: Mycroft Holmes is revealed to be an asset for MI6 and eventually fakes his death to safeguard Sherlock and Joan. At the end of Season 3, Sherlock suffers a relapse following a traumatic case involving Oscar Rankin.
