The yacht. Twelve years before the story, Yeo-ri was on a yacht with Kang Ji-hwan, his adopted older brother Min-hwan, and Park Seung-jae. Ji-hwan gave Yeo-ri a protective pendant, the yacht hit a rock, and he fell overboard. Min-hwan held his hand and deliberately let go after Ji-hwan seemed to realize his feelings for Yeo-ri. Yeo-ri survived and began seeing ghosts.
The heart. Ji-hwan's heart was transplanted into Gang-uk. That explains the deep bond between Gang-uk and Yeo-ri.
Min-hwan's unmasking. Seung-jae finds the camcorder footage and blackmails Min-hwan. Min-hwan kills him and tries to frame Yeo-ri, then sends a dump truck at Gang-uk. Yeo-ri shoves Gang-uk clear and takes the impact herself.
Ending. She survives after a coma, and Ji-hwan's spirit blocks her passage to the other side. Min-hwan ends up in prison, where his mother later visits and says she will wait for him. Yeo-ri's ability disappears, and she and Gang-uk walk off together. Only after the reveal do the earlier scenes of Min-hwan's friendly concern read as possessiveness and concealment.
