The film begins as an apparent paranormal mystery: Riley and her Paranormal Paranoids crew vanish at Shelby Oaks, leaving behind corrupted video clips of an entity.
The first major twist arrives when Wilson Miles commits suicide on Mia’s porch. The unreleased tape reveals that Riley was not killed in 2008 by a specter, but ambushed and abducted. Mia tracks the clues to Norma Miles’ isolated residence. Descending into the underground cellar, the supernatural ghost-hunt transforms into grueling domestic horror: Riley has been alive the entire twelve years, imprisoned in Norma’s dungeon.
The second major twist exposes the sinister purpose behind Riley's captivity. Norma, a devout servant to the horned entity "Tarion" (the demonic figure that stalked Riley and Mia in childhood), orchestrated the kidnapping. Norma forced her mentally compromised son, Wilson, to repeatedly assault Riley in order to produce a child intended as an earthly vessel/sacrifice for the entity.
In the harrowing climax, Mia attempts to rescue Riley, but Riley’s psychological and physical deterioration is profound. Norma and the supernatural entity's presence converge in a brutal confrontation. While Mia confronts Norma and uncovers the horrifying organized photo album documenting the ritualistic abuse, the physical manifestation of the entity / hellhounds appears. Riley dies, and Mia is left shattered in the ruins of the compound. The ending confirms that Mia’s childhood denial cost Riley her life; rather than receiving the fairy-tale rescue she envisioned, Mia is left with an irreversible, nightmare reality.
