The film opens with Frank Castle suicidal at his family's gravesite with a single bullet. Ma Gnucci, seeking revenge for her slaughtered sons, places a massive bounty on his head, broadcasting his location to the city's underworld.
This triggers a 15-minute survival sequence where Frank fights through his apartment building using military tactics, guns, axes, and improvised weapons before escaping onto a school bus. When Ma Gnucci retreats, Frank hears a little girl named Charli crying as armed thugs raid a nearby donut shop. Frank chooses protection over vengeance, abandoning his chase of Gnucci to slaughter the thugs inside and save Charli's family.
Charli gives Frank a handcrafted flower, which he places on his daughter Lisa's grave. Realizing his purpose to protect the innocent, he reclaims his Punisher gear. In a post-climax scene, Frank intercepts Mickey (the thug who killed a homeless man's dog), pins him to a lamppost, stabs his thigh, and executes him with a bullet to the head outside Gnucci's restaurant—confirming the Punisher is fully back.
