In Season 1, Dr. Baek Kang-hyuk arrives at Hankuk National University Hospital to rebuild the severe trauma team. He scouted proctology resident Yang Jae-won, recognizing his talent and potential. Throughout the season, the team faces pushback from hospital director Oh Sung-heum and Chief Hong Jae-hoon, who seek to shutter the trauma center due to mounting financial losses.
In Episode 7, the narrative shifts to an international rescue operation in South Sudan, where Kang-hyuk uses his connections with private military contractor Black Wings to save a critically wounded captain.
In the Episode 8 finale, Chief Hong attempts to revoke Kang-hyuk's medical privileges on charges of administrative malpractice. However, Kang-hyuk uses hospital protocol to override Chief Hong via the Head of Emergency Medicine. Meanwhile, a massive fire breaks out in a nearby building. Kang-hyuk and Dr. Han enter the burning structure, where oxygen tanks are at risk of exploding, to perform triage. Kang-hyuk suffers severe injuries while rescuing a victim. He is rushed back to his own trauma unit, where Jae-won and the team save his life within the golden hour. The season ends with the trauma center securing funding and continuing its mission.
