- Lady Maeda's Betrayal: Lady Maeda is revealed to be the secret financial sponsor behind the hospital's bio-experiments. She deliberately orchestrated Seong-sim's infection out of personal jealousy and revenge.
- Chae-ok's Death & Rebirth: In the Season 1 finale, Chae-ok shields Tae-sang from an attack by the creature. Recognizing her daughter in her dying moments, the creature leaps into the water with Chae-ok and transfers her Najin parasite into Chae-ok's body, bringing her back to life as an immortal host.
- The Time Jump Twist: The post-credits scene of Season 1 shifts to present-day Seoul, showing a young man who looks identical to Tae-sang watching modern TV news, revealing a scar on his neck and the name "Ho-jae".
Season 2 Plot Twists & Series Finale:
- Ho-jae's Real Identity: Jang Ho-jae is not a descendant; he is the original Jang Tae-sang. He had received the Najin in the past to survive, remaining young for decades. A year prior to Season 2, during a fight against Jeonseung Biotech, the Najin was surgically removed from his neck by Kuroko Captain, wiping his memories.
- Jeonseung Biotech's True Form: Jeonseung Biotech was built directly over the ruins of Ongseong Hospital. Lady Maeda and Lieutenant Kato used the company to mass-produce "matured" Najin parasites, creating super-powered, controllable military operatives (Kurokos).
- The Series Finale Resolution: Tae-sang has the Najin re-inserted into his neck, fully regaining his memories and strength. During the final battle, Seung-jo injects Chae-ok with a paralyzing chemical compound that puts her Najin to sleep. Tae-sang breaks the massive water tank where Chae-ok is trapped, washing away the dormant parasite. Chae-ok awakens as a normal mortal human, stripped of her immortality, finally able to grow old and live a peaceful life with Tae-sang.
- The Chilling Mid-Credits Hook: In the final mid-credits scene, Seung-jo takes over operations and mass-distributes Najin larvae inside commercially sold bottled water across South Korea, teasing an impending worldwide epidemic of super-human mutations.
