This breaks Tae-sik, causing him to go on a rampage where he kills Man-seok and his entire crew. Tae-sik then prepares to shoot himself in the head, believing he failed. The Twist: So-mi emerges alive. The henchman Ramrowan had spared her because she showed him kindness earlier, and he shot the jar (which contained the surgeon's eyes) to provoke Tae-sik into killing the gang.
