The central plot twist is that the urgent letter from Yunyun that brings the party to the Crimson Demon Village is a prank, written by a classmate. The supposed 'threat' is initially non-existent. The true threat emerges in the form of Sylvia, a Demon King general who attacks the village seeking a powerful anti-magic weapon called the Mage Killer, created by the same Japanese transmigrator who founded the Crimson Demon Clan.

During a confrontation, Kazuma is captured by Sylvia. He is initially willing to join her due to her beauty, but is horrified to discover that Sylvia, as a chimera, has a male body in addition to her female form. His panicked rejection is a major comedic and controversial scene. Sylvia eventually fuses with the Mage Killer weapon, becoming a monstrous and powerful entity that can nullify the Crimson Demons' magic.

The climax deviates from the light novel. In the film, after a preliminary defeat, Sylvia is resurrected for a larger final battle. The key to her defeat is a magical railgun, another invention of the village's founder. To defeat the empowered Sylvia, Kazuma, with his luck boosted by Aqua, allows himself to be absorbed by her, feigning affection to create an opening. This allows Megumin and Yunyun to combine their power, fueled by energy from all the Crimson Demons and Wiz, to unleash a massive, heart-shaped Light of Saber/Explosion spell that obliterates both Sylvia and Kazuma. Kazuma is, of course, resurrected by Aqua shortly after. In the final scene, Megumin, feeling insecure about her one-track skill set, asks Kazuma to add points to advanced magic on her adventurer card. Instead, he secretly pours all the points into strengthening her Explosion magic even further, reaffirming his faith in her chosen path and making her cry with joy.

The character of Sylvia, the main antagonist, has been the subject of significant debate and varied interpretations. One perspective is that the humor surrounding Sylvia is deeply transphobic. In this reading, Kazuma's initial attraction followed by repulsion upon discovering she has male anatomy is played for laughs in a way that mocks and dehumanizes transgender identities. His negative reaction is framed as the 'correct' one, shared by his party, which punches down and relies on harmful stereotypes.

An alternative interpretation views the situation through the lens of Kazuma's established character as a heterosexual, panicky teenager. This perspective argues that Kazuma's reaction is not a condemnation of Sylvia's identity, but a personal, comically exaggerated moment of sexual confusion and panic from a character who is consistently portrayed as flawed and selfish. It is seen as a reflection of his own hangups rather than a statement on gender identity itself. Some also point out that Sylvia is a 'chimera' who has modified her own body, which complicates a direct one-to-one comparison with human transgender experiences and places the character firmly in a fantasy context where the humor is derived from the absurd situation rather than targeted malice.
